Just found a different variation of the 5th issue Screen Test slot card from the Hollywood Casino in Aurora, IL, the card with "Hollywood" on a film strip and gambling problem phone# is 1-800-522-4700. On the originally listed card the third line of text under the signature strip was "information." while the new found card has "reservations and information."

Basically it's the same text as the previous 4th issue card but with the round logo from the 5th issue cards.
