Turns out there is an earlier version of the purple Club Griffin slot card from Resorts International in Atlantic City. The first one listed previously was a 5th issue with the 1-800-MERV-VIP phone number over the mag stripe. On that card that line of text is 63mm wide and the card I have has a 1997 date on the front. I just found another version of that card that has a 1994 date on the front and the top line on the back is 74mm wide. Additionally, the white outlined box pattern is very faint on this card too. The most obvious place to put it would be 4th issue. Here's scans of the new found card...
