Just discovered was a green Players Club Emerald card that appears to have been issued between what was originally listed as the 14th and 15th issue cards. There was no easy way to add this card without renumbering the 15th issue and make it the 15th issue so the new card could be added. While there, I removed a few previously listed higher level cards from earlier issues that were never confirmed and it appeared they were just added at some time when the lower level cards were added. It looks like it was just assumed all levels were reissued when they may not have been. So they were removed for now and will be re-added whenever anyone reports scans if they're found.

Here's a scan of the new found card... it has the text "Hotel & Casino" under "Wild Horse Pass" wider than the line above like the 14th issue card but "Hotel & Casino" on this card is wider. On the 14th issue card it's 16mm wide and it's 19mm wide on this card.
