Just found a different version of the 3rd issue Club Centaur - Choice level card from Hoosier Park Racing/Casino in Anderson, IN. This is the orange/black card where the last line in the reverse paragraph starts with "Merrillville...".

On the originally listed cards the reverse text is the same size as previous and later card but there is a card with smaller text on the back that would have happened between the listed 2nd and 3rd issue cards. On the new found card the first line of text on the back is 63mm wide while the previously listed 3rd issue card is 73mm wide.
