I found another manufacturing error from the Paradise Casino in Yuma, AZ. The original error was of the 1st issue with no manufacturer's mark. This one is of the 2nd issue with ACC and 8 lines of printed text in the paragraph. It's missing both the magnetic stripe but also the white stripe for the signature strip.
