Looks like there was a mistake in previous issues of the Slot Card Guide for the Triple Play Cards used in Cripple Creek, CO. The manufacturer mark listed for the 14th issue card was cpi 2061617 and I've been looking for one of these for a long time. Well, I finally found one and it appears the manufacturer mark should have been cpi 2071617 so I've made the correction in the new guide.
