Just found a second variation of the Gold level card copyright 2009 from Gold Dust West Casino in Elko, NV. On the originally listed card, the faint "Elko" on the bottom left corner on the back of the card is 30mm wide. On this new found card it's 35mm wide and slightly higher on the back.
